This notice is intended to communicate immediate stop use and inspect information regarding CSA-certified Guardian Fall Protection Halo 20-30 ft. Self-Retracting Lifelines (SRLs) and private label branded versions. A potential isolated manufacturing issue that prevents the locking mechanism from engaging has been identified and requires immediate inspection

There have been no reported accidents or injuries related to this recall.

HOUSTON (March 6, 2020) – Pure Safety Group (PSG) has released an immediate recall on Guardian Fall Protection Diablo Big Block Leading Edge Self-Retracting Lifeline. Due to a potential manufacturing defect over the period of February 2019 through February 2020, this product must be immediately removed from service and returned for inspection and repair.

There have been no reported accidents or injuries related to this recall.

Limited part numbers affected, immediate replacements available

HOUSTON (Dec. 26, 2019) – Pure Safety Group (PSG) has released an immediate recall and stop-use on Guardian Fall Protection Self-Retracting Lifelines for leading edge applications (SRL-LE) that utilize a stainless-steel or web lifeline. A small number of SRLs with stainless steel or web lifelines were identified as non-compliant with ANSI Z359.14-14 and must immediately be removed from service. Under certain conditions, the stainless-steel or web lifeline may not perform to industry standards in leading edge applications and could result in serious bodily injury or death.

There have been no reported accidents or injuries related to this recall. All affected SRLs with stainless steel or web lifelines must be immediately returned to PSG for replacement. Replacements will be made with an equivalent model at no charge.

HOUSTON (Nov. 14, 2019) – Pure Safety Group (PSG) has released an immediate recall on Guardian Fall Protection and Web Device 3- Way Rescue and Retrieval Self-Retracting Lifeline (SRL) units. There have been no reported accidents or injuries related to this recall. This recall affects the following product: Guardian #10974 (50') and #10981 (65'), and Web Devices #0501312 (50') and #0651312 (65'). Any and all custom or private-labeled 3-Way SRLs and 3-Way SRLs sold in Arc-O-Pod kits #s #20001, #20004 are also included in the recall.

Guardian Fall Protection has increased the Competent Person product inspection interval.

Guardian Fall Protection has reduced the fall clearance safety margin from 3 feet to 2 feet.
